Conceptual Framings for Patient Handoffs

In Table 10.1, we provide six alternative conceptual framings and their associated functions, risks, and how the framing tends to point to different types of quality improvement interventions (note that Table 10.1 expands upon initial thoughts about four conceptual framings published in Table 2 of Cheung et al., 2009). These framings are not mutually exclusive, but rather can be viewed as levels of activity that occur simultaneously in parallel during a handoff. Others have made similar distinctions, including Cohen & Hilligoss (2009), who distinguished between correct transition of essential information, transfers of responsibility or control, error correction, and learning at the individual and organizational levels;
